Patient lead through Wheel Chair maneuvering through gym to improve mobility for functional tasks. Patient required verbal and tactile cues with tasks to ensure proper technique and count.
Patient very lethargic requiring frequent stimulation for arousal. Patient has difficulty with accuracy of foot placement. Notable patient's tissue mass can at times make maneuver difficult for patient.
Patient was trained in W/C propulsion and maneuvering within patient's environment forward and around corners in order to improve independent and safe mobility. Patient required maximal verbal, tactile, and visual cueing for appropriate use of push wheels and improved accuracy in propulsion.
In order to increase participation in activities within facility, patient was coached through management and propelling of WC with emphasis on more efficient use of push wheels, safe use of brakes, and maneuvering between/around objects and through doorways. Therapist assessed pt for WC fit with general measurement in bed this day with width of 27 in with need for bariatric reclining wheelchair due to decrease core functional and righting reactions with need for reclining wheelchair to promote posterior lean and reduce fall risk during time out of bed sitting. Noted need for Hoyer lift education for CNA staff to ensure pt safety and increase nursing carry over in facilitating pt time OOB.
