Skilled PT is required as the patient's remaining deficits are presenting at a level of severity or complexity that require the sophistication of a qualified therapist to advance pt through a series of interventions while analyzing pt's response to treatment and modify the program as necessary to ensure the effectiveness of the POC allowing patient to achieve the PLOF of Independent within a reasonable time frame.
Patient requires skilled PT services as the patient's remaining deficits are presenting at a level of severity or complexity that requires the sophistication of a qualified therapist allowing patient to achieve the PLOF within a reasonable time frame to facilitate motor control, and improve mm recruitment in LE, so the patient will be able to perform gait, transfers, at the highest level of independence while analyzing the patients response to treatment and modifying the program as necessary to ensure the effectiveness of the POC allowing patient to achieve the PLOF of Independent within a reasonable time frame.
Continued PT is required as the patient's remaining deficits are presenting at a level of severity or complexity that require the sophistication of a qualified therapist in order to establish and instruct in compensatory strategies, increase independence, improve dynamic balance and increase LE gross motor muscle control in order to enhance patient's quality of life by improving ability to perform gait, bed mobility, and transfers w/increased safety allowing the patient to achieve the PLOF of Modified Independent within a reasonable time frame.
Patient requires the continued level of treatment intensity due to the noted rate and level of progress being achieved that is directly advancing the pt towards recovery of PLOF. Patients current condition is presenting at a level of complexity that is requiring the sophistication of a qualified therapist to promote the pt through the series of physician prescribed interventions with the PT analyzing the pts response to the set treatment regiment and make adjustments as necessary as to insure the effectiveness of the POC and allow the continued recovery to PLOF of MI within a reasonable amount of time.
Skilled PT is required to grade, progress, and modify interventions in real time based on the patient's --- response, which cannot be safely delegated to unskilled personnel.
Skilled PT is required 2/2 --- WB precautions requiring continuous assessment of compliance and immediate correction of unsafe loading.
Skilled PT is required to assess the patient's --- response to activity and to modify parameters accordingly given comorbidities of ---.
Skilled intervention is required to select and progress the appropriate AD as the patient's balance and strength change across the episode.
Skilled PT is required for the assessment of --- and clinical decision making regarding --- , which requires the judgment of a licensed therapist.
Without skilled PT, the patient is at high risk for --- , falls, and a decline requiring a higher level of care.
Skilled PT is required to instruct the patient and caregiver in --- , with real time assessment of technique and correction of unsafe patterns.
Skilled PT is required to establish and progress the HEP based on ongoing assessment of --- ; the program cannot be established without skilled evaluation.
Skilled PT is required 2/2 the patient's --- cognition, requiring continuous cueing and safety supervision that unskilled staff cannot provide.
Skilled PT is required to monitor --- during activity and to determine when parameters exceed safe thresholds requiring termination of the session.
Skilled PT is required for neuromuscular re-education using manual facilitation and inhibition techniques requiring the hands of a licensed therapist.
Skilled PT is required to assess and address the patient's --- gait deviation, which requires movement analysis beyond the scope of unskilled personnel.
Skilled PT is required to progress the patient from --- to --- ; the progression requires continuous assessment of safety and readiness.
Skilled PT is required 2/2 fluctuating status related to --- ; the interventions require adjustment session to session based on presentation.
Skilled PT is required to determine the safest transfer method for this patient given --- , and to train nursing on that method.
Skilled PT is required to assess the patient's --- and to make recommendations to the MD regarding --- .
Skilled PT is required for orthotic assessment, fitting, and skin monitoring, which requires the training of a licensed therapist.
Skilled PT is required to apply and titrate --- , requiring knowledge of contraindications and physiologic response.
Skilled PT is required to advance the patient toward the d/c goal of --- ; the patient is not yet safe to perform the activity without skilled input.
Skilled PT is required 2/2 the complexity of the patient's presentation including ---, ---, and --- , which interact and require ongoing clinical reasoning.
Skilled PT is required to assess and reduce fall risk; the patient's Berg of --- places the patient in the --- risk category.
The patient continues to require skilled PT as evidenced by an assist level of --- for --- and the need for --- percent cueing during ---.
Skilled PT is required to train the patient in --- ; once the patient achieves independence, the program can be transitioned to restorative nursing.
Skilled PT is required to reassess the POC and modify goals as the patient's status changes; the patient is not yet at a maintenance level.
